Every other week, Community Health Alliance steps outside the walls of our clinics and into the heart of Middletown — because access to care shouldn’t depend on whether someone can find us. Sometimes, it means we need to find them.

In early February, Rachel DeWitt, CDCA, our Community Engagement Coordinator, partnered with HopeLine, Full Circle Recovery Services, SOS Wellness, and Haven Behavioral Hospital of Dayton for one of several coordinated outreach efforts that take place each month. Together, the team visited nearly 50 homes, delivering care bags filled with snacks and easy-to-understand information about local services and support.

These outreach days are about more than dropping off resources. They’re about presence, visibility, and connection. Transportation challenges, uncertainty about where to go, stigma, or simply not knowing what services are available can all stand in the way of someone reaching out. By bringing information directly into neighborhoods, we help remove barriers and make the first step toward care feel closer and more accessible.

When providers show up together, it sends a powerful message: support is not scattered — it’s connected. Each organization brings its own strengths, and together we create a stronger network of care for the community.
