Case Management & Peer Support

Comprehensive Support for Your Wellbeing

At Community Health Alliance, we know that healing doesn’t stop when you leave a counseling session. Real progress happens in everyday life—at the doctor’s office, in the courtroom, on the bus, or at home when you’re trying to make ends meet. That’s where case managers and peer supporters come in.

Our case management services are focused on helping you navigate real-life challenges with hands-on support. Whether it’s getting to an appointment, checking in with the court, or making sure your basic needs are met, case managers walk beside you in the community to help you stay on track.

Peer support services offer something just as important: connection. Peer supporters are individuals in recovery themselves—people who’ve faced similar struggles and now offer encouragement, motivation, and practical wisdom as you move through your own journey.

Together, case management and peer support form a bridge between clinical care and daily life—so you don’t have to figure everything out on your own.

What It Can Look Like

Need to get to court, a medical visit, or have your medication when needed?
Case managers support you through health care, legal matters, or staying on track with recovery – we can help you follow through.

Case managers assist with medication check-ins, refills, and coordination with prescribers—especially for individuals managing multiple medications.

Through access with our partner resources, TLC staff help ensure you and your family have what you need to stay healthy and stable.

Facing probation, child support court, or housing-related legal issues?
We provide coordination and accountability while supporting your dignity and progress.

Case managers can meet you in the community—not just in an office—making care more flexible, responsive, and personal.

Peer supporters offer a judgment-free space to talk, vent, celebrate, or cry. They share real-life insight and help you stay focused on your goals, stay motivated, and keep you on track even when things get tough.

Whether you’re in early recovery, facing relapse, or just having a bad day, peers provide a lifeline when you need someone who “gets it.”
If you need to talk, we’re available 24/7 just by dialing 988.

Case management and peer support span across multiple locations throughout the region—including referral-based residential housing, outpatient locations, and recovery housing locations. For a comprehensive map of all our locations, please refer to our Locations page.

Our Providers Can Help With

If you’re overwhelmed or disorganized, case managers can help structure your week, provide reminders, and
even help with transportation.

We won’t let you go without. Case managers connect you to emergency resources and coordinate with CHA’s food pantry and community partners.

If you’re stuck in paperwork or don’t know where to start, case managers will walk you through benefits applications, court paperwork, and follow-up steps.

When you feel like no one understands what you’re going through, peer supporters step in as a voice of experience and encouragement.

If you’ve hit a rough patch or slipped, peers help you refocus without shame. They remind you of your goals and support you in getting back on track.

If you’re working, parenting, in school, or managing housing stress while in treatment, we’ll help you plan, prioritize, and move forward with support.

Case managers provide structured reentry plans, and peer supporters help navigate emotional challenges and connect to resources for a fresh start.

Whether it’s a court date, intake appointment, or hard conversation, our case managers and peers are available to be by your side—supportive, steady, and in your corner.
