Comprehensive Support for Substance Use Recovery

At Community Health Alliance, we understand that recovery looks different for everyone—and that healing from substance use requires more than willpower alone. If you’ve been searching for rehab near me or addiction treatment near me, you’ve already taken the first step toward healing. We’re here to walk with you the rest of the way.

Our Sojourner Recovery Services program offers compassionate, evidence-based care to support you at every stage of recovery—whether you’re just starting out or maintaining long-term sobriety. Sojourner is a trusted addiction treatment provider in Ohio, offering a full continuum of care including outpatient treatment, residential rehab programs, Medication-Assisted Treatment (MAT), peer support, case management, and sober living.

Our approach is integrated and person-centered, connecting medical care, mental health support, and daily life needs into one cohesive plan for lasting recovery. Wherever you are in your journey, we’ll meet you there. You don’t have to face addiction alone—and you don’t have to wait for a crisis to ask for help.

What It Can Look Like

For individuals living with substance use concerns, Medication-Assisted Treatment (MAT) offers a supportive bridge to healing. Offered through Sojourner Recovery Services, MAT is often coordinated with TLC’s behavioral health and psychiatric services.

Offered at both Sojourner and TLC, this level of care provides a structured, supportive environment where individuals can safely manage withdrawal symptoms.
This service aligns with the ASAM Criteria, a nationally recognized framework from the American Society of Addiction Medicine and is specifically tailored to the individual and their needs.

Our 24/7 residential treatment program supports people experiencing serious
challenges with both mental health and substance use. This structured level of
care focuses on stabilization, healing, and preparing for long-term recovery in the
community.

If medication or a deeper evaluation is needed, our psychiatrists are here to help.
We work with you to understand what’s going on and support you in finding the
right balance.

Part of our wraparound services, peer recovery specialists offer lived experience, encouragement, and practical guidance for staying on the path of recovery.

Talk one-on-one with a licensed therapist in a private, judgment-free space.
Together, you’ll work on what matters most to you—building coping skills,
navigating tough emotions and moving toward the life you want.

You’re not alone. Group sessions bring people together to share experiences, learn from one another, and build a sense of connection. A trained professional leads each group in a supportive and safe space.

Case management helps individuals stay connected to care and meet personal goals while living in the community.
Our case managers can assist with benefits, housing, appointments, and anything else that helps you stay on track.

Our integrated medical services support everything from routine care to medication monitoring—ensuring you feel well in every way.

TLC partners with local courts, jails, and community agencies to provide support for individuals navigating the justice system, including jail-based services and discharge planning.

We help individuals overcome barriers to employment and schooling—offering
support with training, applications, and confidence building along the way.

We provide tailored care for pregnant women, people experiencing
homelessness, those reentering the community after incarceration, and more.
Whether your need is clinical, social, or practical—we meet you where you are.

We offer a range of addiction recovery services across multiple locations throughout the region—including referral-based residential treatment, outpatient support, medication-assisted treatment, and recovery housing. For a comprehensive map of all our locations, please refer to our Locations page.

If you’ve had a setback, we’ll meet you with compassion—not judgment—and help you re-center your goals and move forward with strength and support.
